To support the learning and mental health needs of the service user group and enable individuals to lead a meaningful and worthwhile life within the community giving practical help, care, guidance and support as detailed within the service user’s individual care plans and risk assessments.
You will be expected to work a flexible shift system, which may include sleep-in duties or waking nights depending on your contract and to assume a level of responsibility designated by the Registered Manager. You will arrive for work on time and comply with the organisations policies and procedures. You will participate in assessing and planning to meet individual’s needs as well as to participate in meetings with other professionals and service user’s families. To plan effectively for a positive future and be part of the development of a realistic care plan. You will participate in assessing and planning to meet individual’s needs as well as to participate in meetings with other professionals and service users families. To support service users in attaining a positive future, promoting independence and develop a realistic care plan in order for the service users to achieve realistic goals and targets. To support the Management team in developing and completing assessment tools with the aim of being able to provide and meet a wider range of services for the individuals in our care. To be proactive in managing service users with complex enduring needs. You will be expected to participate in regular supervisions and take responsibility for your personal development. You will be required to attend training programmes in order to develop your knowledge and skills which will be identified by you personally and your Management team.
You will, be expected to show a level of commitment and initiative and forge links with community facilities, to enable your respective service user group to develop their potential skills in all areas. You will be expected to familiarise yourself with CTTM Policies and Procedures and all legislation relevant to the care sector. You will be expected to report all accidents/incidents and/or other health hazards in line with CTTM policy. You will maintain confidentiality in all aspects of service user care and handling of financial accounts and to ensure that all documents and records you write are legible, accurate, concise and written in black ink.
You will be expected to provide a high level of dignity, respect and value in all areas of contact, whether in the home or accompanying people in the community and to assist in the day to day maintenance of the home, i.e. purchasing food, supporting service users with personal care and maintaining the home environment to a high standard.
